LODGE: The medical report is to be completed by your family physician and returned to us with the general application form along with a copy of your current tax notice of assessment. The notice of assessment is for income verification to see if the applicant qualifies for subsidy. The application package can be returned to us either by fax, mail or in person.

SUPPORTIVE LIVING: The supportive living application must be returned with the reference letter from the Alberta Health Services, stating that the applicant has been approved for this program. Please contact Alberta Health Services as well as the Piper Creek Foundation for supportive living needs.

After we have received the completed application package, one of our Lodge Managers will contact you (in 2 weeks time) to arrange for an assessment interview. This interview will determine where the applicant will be placed on our wait list. This is not a 1st come-1st serve basis. Our wait list is based on an individual’s needs, which are determined by our point score system at the time of the interview. The applicant’s name will then be added to our wait list and their position can be monitored by the applicant or family through the Foundation office. The higher the score-the sooner the placement.
