To apply for our programs, there are application packages that must be completed. The application may include the application form and a medical report, where as the supportive living application only includes the general application. These packages can be obtained by following the links, or we have copies at our office that can be picked up, mailed or faxed to your residence. The instructions for completing these applications are listed below.

 

 

LODGE:  The medical report is to be completed by your family physician and returned to us with the general application form along with a copy of your current tax notice of assessment.  The notice of assessment is for income verification to see if the applicant qualifies for subsidy.  The application package can be returned to us either by fax, mail or in person.

    SELF CONTAINED (APARTMENT LIVING): 

   The self contained application must be returned with the medical report which is to be completed by your family physician and returned to us with the general application form along with a copy of your current tax notice of assessment.  Our waitlist is based on an individual's needs, which are determined by our point score system after all the forms have been received.
